• Fixed: Sabre_DAV_Locks_Backend_PDO returned too many locks.
• Fixed: Sabre_HTTP_Request::getHeader didn't return Content-Type when running
  on apache, so a few workarounds were added.
• Change: Slightly changed CalDAV Backend API's, to allow for heavy
  optimizations. This is non-bc breaking.
