To monitor more with more detail disks on a Linux machine, please follow this repo : https://github.com/grundic/zabbix-disk-performance
If you prefer smart analysis of yours disks : https://github.com/v-zhuravlev/zbx-smartctl.git
But you might need to modify the files. It didn't work at first try for me.
