Follow up to #5060

When the "claim" mapping strategy is used, authentication can fail if a user with the preferred username is already mapped to a different identity. In that case, a cluster admin may want to switch to the add or generate strategy. We should ensure the authentication error is logged appropriately, and possibly call out the existence of multiple "claim" strategies with a warning or info in diagnostics.
