"upserted" should be removed from the UI / UX

This command:

$ tctl create -f r.yaml

Keeps printing:

role intern upserted

Also, "upsert" behavior is causing issues elsewhere for us. Lets remove "upsert" semantics from everywhere reasonable (i.e. where real world users can see it) and use a proper "create" and "update".

There is tctl update, BTW.
