@keberlein , @infotexture ,

Here are a couple incremental commits for the Creating plug-ins section. I'm still working on the new/revised topics for creating new extension points, but the two commits here I believe provide some key information about creating plug-ins that's been missing to date.

I'm looking forward to your suggestions, revisions, or later improvements to this information.

* Reorganize content
* Some corrections
* Expanded descriptions
* Examples

@shaneataylor Thanks, nice improvement.

I was able to merge this because the develop branch in your fork had not advanced further since you created the pull request, but in the future, please remember to create a topic branch in your fork and issue the pull request from there to ensure the changes in the pull request remain isolated from revisions to the main development stream.

I removed an unresolved keyref & fixed a typo in 3505f72.

Feel free to restore the keyref in your next PR.
