Improve the issue templates

Description

Updates the feature request and bug issue templates to be a bit easier to use for the author and to have better formatting for the reader.

• Adds comment instructions for the author
• Adds additional formatting around titles
• Moves community note to bottom with more eye catching formatting
• Adds bug or feature request to the planning project automatically

Validation

• Try the templates in GitHub
• Ensure the project is added properly
