Update the issue templates to use form templates

PR Summary

Update the issue templates to use form templates.
Also request more relevant information when the user is facing an exception report.
Bug_Report.md is internationally kept because recent PSReadLine releases all print https://github.com/PowerShell/PSReadLine/issues/new?template=Bug_Report.md for the user to report an issue when exception happens.
