A long standing issue when developing Q2 mods has been lack of access to server VFS. This fixes it.
Will be shared with game. Always use 64-bit time in file_info_t. Change FS_MODE_READ to 0. Add more comments.

Upstream a mechanism to provide an extended game API that's still compatible to the vanilla binary interface.
