Description

Porting bug fixes to 4.0 branch and changing version for 4.0.1 release

#3719
#3714

Motivation and Context

product support finished last week this releases the bug fixes from that work.

How Has This Been Tested?

Nightly tests passed on all tests passed on 4.0 branch (except for expected failures from feature changes).
