apache license appendix should not be substituted

I believe the Apache license appendix is supposed to have those placeholder looking brackets like [yyyy] and [name of copyright owner]. I'm no expert, but I'm just going off of what the text of the appendix says and the fact that when you let github create an Apache license for you it comes with the bracketed portion included.
