Interactive PDF doesn't work

How to reproduce this problem:

• Open http://www.jasnapaka.com/temp/INteractivePDF.pdf.
• There are some blue points (cities) and hover over these points should open bubbles with images. Try in Adobe Reader. With pdf.js are these images showed by default.

Probably an annotation issue. Marking as such for now.

It's also strange that the form warning bar appears here. @brendandahl?

It's probably using acrobat forms to create this affect. BTW pdf.js exhibits the same behavior as OSX preview.
