[$apply] Adding the new Plan Definition, CarePlan and Task Resources as test cases.

• This is related to the $apply implementation. Adding these files helps us test the $apply further against real-life situations.

Expected Outcome

• The CarePlan & Tasks should be generated as part of the $apply test cases.

Those test cases started my journey in running the CQL Compiler on Android. With the Translator 2.0.0, we should be able to run those test cases. Without 2.0.0, we need to adapt the PlanDefinitions to remove conditionals (which need to be turned into CQL and compiled).
