Allow to define index settings per index

At the moment we can only create a mapping for a specific index but sometimes we also want to specify index settings. This is possible per challenge but does not always make sense (e.g. consider using different indices in one challenge).

We should allow for more flexibility here, probably by just issuing a create index API call (combining settings + mappings).
