[Self-hosted] Instruction link in Unauthorized 403 page is wrong

Describe the bug
The link that says "click here to activate your account" points to
https://codecov.self-hosted.com/members/github_enterprise/org
instead of
https://codecov.self-hosted.com/account/github_enterprise/username

Environment (please complete the following information):

• Browser: Chrome, perhaps all
• Codecov Version: 24.10.1, 24.11.1

To Reproduce
Steps to reproduce the behavior:

1. Diactivate your seat in Codecov
2. Land on a private repo page
3. Get the Unauthorized 403 page
4. Click on the link that says "click here to activate your account"
5. Go to the wrong page

Expected behavior
Go to the correct page. But in this case your seat should be auto-activated - see #554
