Beginner's grace failure when exit to title

Describe the bug
When I died in my second vault, I quit to title instead of respawning and shut down the game. On return to the world, my inventory was empty and was not filled with the stuff I had pre-vault

To Reproduce
Steps to reproduce the behavior:

1. Enter beginner's grace vault with some items in inventory
2. Die
3. Select "exit to main menu option on death screen"
4. Relog into world to see items gone

Expected behavior
Have pre vault loot upon logging back into game
