No Teleport on Vault Completion

Describe the bug
Had a Scav vault that when i went to put the the last items in (Blood Vials), it completed it. but then still said i needed the Blood Vials and when i tried putting more in it just kept spitting them out.
It would not teleport me out of the Vault but the timer was also frozen, which might would let me have infinite time in vault but i was to scared to check lol. Also gave no Crate
Also after this i did a Vault with a boss, got all the obelisks, killed the boss and it said "Killed Boss" (Forget the exact wording) but no teleport timer and i never teleported and had to run all the way back to the portal to leave and once again the timer was frozen. No Crate

To Reproduce
Steps to reproduce the behavior:

1. Enter Vault
2. Finish Vault Quest
3. Timer Freezes and never get teleported out of vault
4. Infinite time in Vault?

Expected behavior
I would expect once I either deposit the last Scav item or kill the boss it would teleport me out of the Vault and award a Crate.

Additional context
I have custom edited some of the vault configs and added a few mods but nothing that i could think would cause this.
